Program Guidelines
- This plan is honored only at Pure Smiles Dentistry. It is NOT an insurance plan that can be used at any other dental office.
- Your effective date is the day you sign up and your renewal date is the same date every year
- Automatic renewal with credit card on file, unless cancelled prior to expiration date
- Only eligible for patients without dental insurance
- Benefits cannot be transferred to other family members or individuals
- Rates are subject to change annually
- Membership fees are non-refundable
- Discounts shall not apply to any treatment started prior to enrollment or after membership expires
- Discounts shall not apply to treatment paid in full or in part by an insurance policy
- Patient’s portion of any bill is due on the same day as service
- Discounts cannot be combined other special offers or discounts
- Excludes all retail products
