- This plan is honored only at Pure Smiles Dentistry in Marietta, GA. It is NOT an insurance plan that can be used at any other dental office.
- Your effective date is the day you sign up, and your renewal date is the same date every year.
- Automatic renewal with credit card on file, unless canceled prior to the expiration date.
- Only eligible for patients without dental insurance.
- Benefits cannot be transferred to other family members or individuals.
- Rates are subject to change annually.
- Membership fees are non-refundable.
- Discounts shall not apply to any treatment started prior to enrollment or after membership expires.
- Discounts shall not apply to treatment paid in full or in part by an insurance policy.
- Patient’s portion of any bill is due on the same day as service.
- Discounts cannot be combined with other special offers or discounts.
- Excludes all retail products.
